Q: What
are your fees for each treatment?
A: You can pay more elsewhere. We are confident
that when you consider the quality of care provided, you will discover
that we have one of the most reasonable fee schedules offered in California
today. They are:
- Private
pay fee for an acupuncture office visit: $40.00
- Private
pay fee for a chiropractic office visit: $40.00
- Adult
rate, or office visit with physical therapy: $60.00
- Chiropractic
and Acupuncture: $80.00
- One
modality of physical therapy: $20.00
Q: What
insurance carriers do you accept?
A: As of April 2002, Dr. Walt is a preferred
provider with Affordable Medical Network, Blue Shield, C.C.N., and First
Health Network. He is not a preferred provider with any H.M.O. Insurance
company, including A.S.H.N. (The insurance alliances may be subject
to change at any time, as the insurance companies frequently change
their coverage policies.)
Q: What
is your automobile accident insurance financial policy?
A: California laws provide for full payment
of treatment for injuries sustained as a result of an auto accident,
if you have liability coverage on your insurance policy. If the other
car was at fault, their liability policy can eventually pay for your
treatment. You are also covered under the Medical Pay and Uninsured Motorist
Clause of your own policy.
Each
office visit consists of a limited exam, treatment and physical therapy.
This takes about a half-hour of your time.
Q: What
is your workers compensation insurance financial policy?
A: California laws provide for full payment
of First Aid or Emergency Treatment for injuries sustained while on-the-job.
Further treatment must then be authorized by either the employer or the
insurance carrier.
Each
office visit consists of a limited exam, treatment and physical therapy.
This takes about a half-hour of your time.
Q: How
much will I be reimbursed under Medicare Insurance?
A: Medicare has a $100.00 deductible, and
usually reimburses for 12 office visits per diagnosis. Dr. Walt is
not a participating provider with Medicare. You are charged the usual
private pay fees, and we will bill Medicare for you. As of April
2002, most patients are reimbursed $36.00 per visit.
Many
Secondary/Supplemental insurance completely invalidate the above
reimbursement rate. You need to check with your secondary to find
out if it invalidates Medicare's reimbursement for Chiropractic.
Q: Do
you cover personal injury liens?
A: Most personal injury liens are not accepted.
Dr. Walt can answer questions regarding your treatment options, or your
attorney may refer you to a health care practitioner.
